Job summary

Qualifications

• Experience in supported housing or social care.
• NVQ Level 2/3 in Health & Social Care or equivalent.
• Full UK driving licence and access to a vehicle.

• Assist individuals with housing, health, benefits, employment and community services access.
• Conduct risk assessments, support plans, and outcome-focused reviews; maintain records.
