Job summary

West Northamptonshire Council is seeking an Adult Social Care Worker to help adults live independently, work with families and carers, and connect people with the right support. You'll join a supportive Adult Services team and work across community-based services, hospital discharge and transition pathways.

You will carry a caseload, complete assessments and support plans, safeguard vulnerable people, and collaborate with health, social care and partner organisations to achieve positive

• Strong communication and listening skills with families and carers.
• Ability to manage own workload and make decisions in the person’s best interests.
• Experience or understanding of adult social care legislation and safeguarding.

Responsibilities

• Work alongside adults, their families and carers to understand what matters most to them.
• Complete assessments, reviews and support plans that focus on people’s strengths, goals and independence.
• Manage your own caseload and respond to changing needs, including supporting people during difficult or challenging situations.

• Identify safeguarding concerns and take action to help keep people safe.
