Job summary

Transforming Support is seeking a dedicated Support Worker in Peterborough to help an adult with complex needs live more independently. You will provide person-centred support in the client’s home, helping with daily living skills, routines, and community activities.

The role requires at least six months’ care experience and the right to work in the UK, with full-time permanent employment, 38.5 hours per week, and a two-week rolling rota.

Qualifications

• You will need at least six months’ previous experience in a care or support setting.
• You must already have the right to work in the UK, as we are unable to offer visa sponsorship.

Responsibilities

• Providing respectful, person centred support within the individual’s own home.
• Helping them develop daily living skills and build confidence and independence.
• Supporting informed choices about their routines, lifestyle and future.
• Helping them manage their tenancy, household responsibilities and finances where required.
• Supporting health needs, appointments and medication in line with agreed procedures.
• Encouraging community involvement, relationships, personal interests, education and employment.
• Contributing to support plans, assessments, reviews and positive risk management.
• Maintaining clear, accurate and objective records.
• Following safeguarding, confidentiality and health and safety procedures.
• Working closely with colleagues and professional teams to provide consistent support.
