Job summary

Transforming Support in Peterborough is hiring full-time permanent Support Worker roles to help adults with complex needs live with greater independence. This role offers a supportive team, a regular 38.5-hour week, and a focus on person-centred care.

You will build trusted relationships, support daily living, manage routines and finances, and participate in community activities to help individuals achieve their goals.

Qualifications

• At least six months' experience in care or support.
• Commitment to helping someone live a fulfilling and independent life.

Responsibilities

• Provide respectful, person centred support within the individual's own home.
• Help develop daily living skills and build confidence.
• Support informed choices about routines, lifestyle and future.
• Assist with tenancy, household tasks and finances as needed.
• Support health needs, appointments and medications per procedures.
• Encourage community involvement, relationships, personal interests, education and employment.
• Contribute to support plans, assessments, reviews and positive risk management.
• Maintain clear records and follow safeguarding, confidentiality and health and safety procedures.
• Work closely with colleagues and professional teams for consistent support.
