Job summary

Case Management Cymru Ltd. is seeking a compassionate Support Worker to assist a client with brain injury in daily activities, shopping, and attending a day centre on Tuesdays. You will provide practical support at home and help plan weekly routines.

The role requires warm, patient, and reliable qualities, with excellent communication and ability to work as part of a small multidisciplinary team. You must have access to a car and be willing to undertake an Enhanced DBS check.

Qualifications

• Experience with brain injury clients
• Excellent communication skills
• Warm, chatty, patient and reliable

Responsibilities

• Provide support to attend day centre sessions and local activities
• Assist with home admin: planning, shopping lists, appointments
• Support to access facilities in the local area (shopping, coffee/lunch, volunteering)
