Job summary

Case Management Cymru Ltd. is seeking a warm, patient Support Worker to provide dedicated, person-centred support for a gentleman with a brain injury. You will build a positive trusting relationship, motivate independence, and help access meaningful activities within a multidisciplinary team.

A full induction and DBS Enhanced check will be provided; driving is essential as you may drive the family vehicle and visit community activities. Mileage is paid at 45p per mile.

Qualifications

• Be able to drive (full UK driving licence).
• Be willing and able to drive the family vehicle.
• Have experience of supporting a person with a brain injury.
• Enjoy working as part of a team.
• Be willing to register with Social Care Wales.

Responsibilities

• Take a ‘professional buddy’ approach, building a positive relationship while motivating and encouraging the client to achieve his goals.
• Support him to engage in meaningful activities, develop independence, and access opportunities that enhance his quality of life.
• Provide a full induction and training.
• This role is subject to an Enhanced DBS check.
