Job summary

Case Management Cymru Ltd. is seeking a dedicated carer to support a private client in Cardigan. You will assist with personal care, help the client stay active in interests and navigate daily life, while coordinating with a multidisciplinary rehabilitation team.

Applicants should be calm, patient, reliable and consistent, enjoy getting to know the client, and participate in decisions to promote independence. DBS check and full induction provided.

Qualifications

• Be able to drive and have use of a car.
• Willing to transport the client using the client’s vehicle.
• Has experience supporting someone with a brain injury (desirable).
• Enjoy working as part of a multidisciplinary team.

Responsibilities

• Provide personal care as part of the role.
• Support the client to access the community and enjoy interests.
• Work with the rehabilitation team including physiotherapist, occupational therapist and neuropsychologist.
• Be a positive, consistent part of the care team to maintain quality of life.
