Job summary

Achieve together is seeking compassionate Support Workers to help people live independently, with training provided to develop skills and confidence. The role focuses on delivering person-centred care, facilitating daily enrichment, and supporting community engagement.

You will work as part of a team, maintain records, and uphold safeguarding with a DBS check. A role in deaf services with communication support may be involved.

Qualifications

• Care experience is beneficial but training is provided.
• Passionate, hard‑working and empathetic outlook desired.
• DBS check required before/after start.

Responsibilities

• Enhance quality of life for people we support and help them achieve goals and life skills.
• Maintain health, wellbeing and personal care; accompany health appointments and administer medication per policy.
• Follow health and safety, safeguarding policies, report concerns promptly.
• Encourage participation in community activities and opportunities to maximise inclusion.
• Support the people we support to stay connected with family and friends and participate in holidays.
• Maintain accurate records: care plans, risk assessments, daily reports and incidents.
• Use preferred communication methods for individuals and respect communication needs.
• Work as part of a team; attend meetings and share feedback.
• Support behaviours that challenge safely using trained approaches.
