Responsibilities

• Build meaningful relationships as an associate key worker.
• Support person-centered care plans tailored to each individual.
• Help assess and manage risk, health, and behavioural changes.
• Encourage friendships, social activities, and community engagement.
• Assist with personal care, household tasks, and medication administration.
• Escort service users on outings and appointments.
• Maintain accurate records and uphold financial integrity.
• Champion dignity, respect, and inclusion at all times.
