Community Integrated Care is seeking a compassionate Support Worker to assist a gentleman in his late 60s who lives at home. You will provide person-centred support with daily living tasks, promote independence, and help him access local community activities and services.
The role includes day and night shifts (30 hours per week), driving the team vehicle, assisting with personal care, meals, mobility, and ensuring a safe, welcoming home environment.
Working closely with the Service Leader and the Support Worker team, you will be responsible for supporting a gentleman in his late 60s. The individual has dementia, diabetes, and a mild learning disability and is supported to live as independently as possible within his own home.
The role involves providing person-centred support with daily living activities, promoting independence, and enabling access to local community activities and services.
Shifts: 8am till 10pm Shift Requirement: Days and Nights Contract: 30 Hours UK Driving License Required? Full UK Manual Drivers Licence essential
