Live Connected is an exciting branch of the Connected Health group. We provide quality care to adults with mental ill health, learning disabilities, dementia, and complex needs, through supported living.

The shift patterns may vary and include: Days, Nights, Evenings, Weekends

KEY DUTIES AND RESPONSIBILITIES

  • Undertake daily living tasks as agreed with the service user, their family, and involved professionals.
  • Work within the policies and procedures of Connected Living.
  • Follow care and risk assessments tailored to each service user's needs.
  • Complete Health and Safety checks in line with policies.
  • Confidently work alone as per lone worker risk assessments.
  • Ensure safe medication delivery and management at service users' homes.
  • Respect service users' preferences in task execution.
  • Promote choice and independence for service users.
  • Manage behaviors in the least restrictive manner.
  • Complete all documentation and provide high-quality handovers at the end of shifts.
  • Support clients' emotional and physical wellbeing as part of a team and wider MDT.
  • Develop and maintain professional relationships with service users and multidisciplinary teams to ensure positive outcomes.
