Job summary

Choice Support is seeking a dedicated Support Worker to provide person-centred assistance to individuals with varied needs. You will help with daily living activities, promote independence, and support wellbeing in a respectful, empowering way.

You will accompany people on walks, trips, and community activities; encourage social participation; and work with PBS plans to ensure consistent, compassionate support aligned with each person’s goals and preferences.

Job description

As a Support Worker, no two days are the same. You will provide person-centred support tailored to each individual's needs, preferences, and aspirations.

Your responsibilities will include:

• Supporting individuals with daily living activities and promoting independence.
• Encouraging community participation and social inclusion.
• Accompanying individuals on long walks, swimming sessions, church services, college placements, shopping trips, holidays, and day trips.
• Supporting people to attend social events, including clubs, nightclubs, and other leisure activities.
• Supporting individuals with Positive Behaviour Support (PBS) needs and following support plans effectively.
• Promoting choice, dignity, respect, and wellbeing at all times.
