Job summary

Rehability UK Support Services Ltd is seeking a compassionate Support Worker to help individuals achieve greater independence and manage daily living tasks with personalised assistance.

You will support with personal care, meal planning, appointments, and social activities, while maintaining dignity and privacy. Training will cover privacy, safety, and medication procedures, with a focus on inclusive, person-centred care.

Responsibilities

• Providing person-centred support tailored to individual needs.
• Encouraging and supporting people to participate in meaningful activities and hobbies.
• Supporting individuals with community access, shopping, appointments, and social activities.
• Assisting with personal care where required.
• Administering medication following training and company procedures.
• Supporting with meal planning, cooking, household tasks, and maintaining a safe living environment.
• Assisting individuals to attend medical and healthcare appointments.
• Promoting independence, dignity, choice, and inclusion at all times.
• Maintaining accurate records and working collaboratively with colleagues and healthcare professionals
