Job summary

Rehability UK Support Services Ltd is seeking a dedicated Support Worker to help individuals gain greater independence and manage daily living tasks. You will provide person-centred support, promote dignity and choice, and enable participation in activities and community engagements.

Daily duties include assisting with personal care, medication administration after training, meal planning, and coordinating healthcare appointments while maintaining accurate records and collaborating with care

• Maintaining accurate records and working collaboratively with colleagues and healthcare professionals
