Job summary

Havebury Homes is seeking a committed Support Worker at Lucy Adams House to deliver housing related support and empower residents on their journey to independent living. You will complete risk assessments, tailor support plans, and collaborate with partner agencies to meet needs and progress to independent accommodation.

Job description

We are looking for a committed and person-centred Support Worker to join our team at Lucy Adams House, delivering high-quality housing related support to residents on their journey towards independent living. You will complete individual risk assessments and tailored support plans, work closely alongside partner agencies to ensure all support needs are met, and play a vital role in empowering residents to build the skills and confidence they need to move on successfully to independent accommodation.

We own and manage c.8,000 homes in the east of England, working closely with other agencies and stakeholders within our communities. We build on the good foundations we have in place in being a customer‑centric organisation, investing in our existing homes - including our zero‑carbon journey - as well as building new homes.

Requirements

Outline of key responsibilities...

  • Deliver a person-centred approach when developing, implementing and reviewing support plans and risk assessments, ensuring full compliance with organisational policies and procedures
  • Promote equality, inclusion and fair access throughout the service, empowering residents to maximise their potential and achieve positive outcomes
  • Contribute to the assessment and onboarding of new referrals, including completing initial risk assessments and liaising with relevant external partner agencies as part of the decision-making process
  • Encourage and support residents to access appropriate services identified within their support plans, including completing referrals and providing effective signposting to internal and external resources
  • Build and maintain strong working relationships with statutory and voluntary agencies to enhance and complement the support available to residents
  • Support residents to sustain their current accommodation and prepare for future housing opportunities by developing life and social skills, ensuring access to eligible benefits and assisting with social housing register applications and the bidding process
We are looking for someone who has/is...
  • A valid full UK driving licence and access to their own vehicle
  • A good knowledge and understanding of the issues that can lead to homelessness, and the complex challenges faced by individuals in housing need
  • Experience of working with individuals experiencing mental health, drug or alcohol related issues, with the ability to respond with empathy, professionalism and resilience
  • A clear understanding of safeguarding responsibilities and the ability to identify, record and elevate concerns appropriately in line with relevant policies and procedures
  • A genuine commitment to promoting equality and diversity, with the confidence and sensitivity to interact effectively with people from all backgrounds and walks of life
  • A passion for supporting individuals to develop the independent living skills they need to sustain their own accommodation and build a positive future

  • Flexible working around a 37 hour week
