It is an exciting time to join our expanding rapid response and intermediate care teams within a Support Worker/HCA role. This role involves working as part of a multidisciplinary team with Nurses, Occupational Therapists, and Physiotherapists.
The team aims to support hospital discharges, prevent admissions, and help people stay as independent as possible in their homes through rehabilitation programmes.
Our base office is at Sheppey Community Hospital; however, you will be provided with equipment to enable mobile working. Virtual handovers are now undertaken remotely to maximize productivity.
This role is suitable for candidates with NVQ Level 3 or equivalent qualifications/experience. Strong communication and organizational skills, along with flexibility, are essential to deliver positive patient experiences.
You should be able to drive and have access to a car to travel across services efficiently.
Proficiency in IT systems and working knowledge of Microsoft Office (Outlook, Word, Excel) are required.
