Job description

Empower Lives. Build Confidence.

At Hft, a leading Social Care provider, we stand alongside learning disabled adults, empowering them to live life their way. As a Support Practitioner specialising in Autism, you’ll play a vital role in creating opportunities, building confidence, and helping people achieve their goals in everyday life. Every day will be different: from cooking meals and planning weekly shops, to outdoor adventures, live gigs, or simply building confidence in public transport. What matters most is that you’ll be there to enable choice, dignity, and independence — supporting people to live the life they choose.

What You’ll Be Doing

Every day is different, but your role focuses on empowering independence, choice, and dignity:

  • Enable people to live with independence and dignity, offering support that builds skills, confidence, and achievement.
  • Provide thoughtful, person-centred care; from daily routines and personal support to helping people take on new challenges.
  • Create meaningful opportunities for people to discover hobbies, learn new skills, and stay connected with family, friends, and community life.
  • Support individuals to manage everyday tasks like cooking, shopping, budgeting, and travelling, so they feel in control of their own lives.
  • Use your experience with Autism and behaviours that challenge to create safe, supportive environments where people can truly thrive.
  • Role model Hft’s values; kindness, respect, and inclusion; and inspire others to do the same.
  • Work flexibly, including sleep-ins, ensuring people have the right support whenever they need it

  • Flexibility to work evenings, weekends, waking and sleep-in shifts
Life at Hft
