Job summary

You Matter is seeking a Support Practitioner to join our team at Alt Bank House. You will provide empathetic, practical support to residents experiencing homelessness and other complex needs, helping them access services and make positive changes.

In this role you will run person-centred sessions, manage risk assessments, support tenancy readiness, and coordinate with social workers and other professionals to deliver tailored support plans.

Qualifications

• Experience of supporting a caseload of clients (homelessness / addiction / mental ill health / offending behaviours).
• Knowledge of external services and Property Pool Plus.
• Ability to communicate in house and with external services both verbally and in written form.
• Ability to put yourself in the place of others and respect their journey.
• Resilience and courage to maintain challenging work.
• Ability to understand and work within Microsoft Word and Excel.

Responsibilities

• Carry out new service user welcomes.
• Ensure service users understand their rights and responsibilities during their stay.
• Support service users to achieve goals defined in their support plans.
• Update and communicate service users risk assessments as per policy.
• Ensure service users shape their own support and understand its purpose.
• Hold planned key-work sessions weekly using a person-centred reflective approach.
• Assess support needs via Person Centred Support Plan and ITEP mapping; plan around individual needs.
• Maintain up-to-date records on MainStay and DAVE systems.
• Conduct bi-monthly risk assessments and inform relevant parties of changes.
• Support future tenancy planning to promote independent living.
• Motivate and challenge service users to achieve goals.
• Liaise with external agencies for specialised support as appropriate.
• Encourage involvement of service users in meetings and forums.
• Deal promptly with complaints and safeguarding disclosures.
• Demonstrate creativity in overcoming obstacles and commitment to vulnerable people.

Salary: £30,000 per annum

Hours: 37.5 hours per week (5 days a week on a 5 week rolling, mix of earlies and lates)

Your role as a Support Practitioner will be to create a safe environment that empowers residents to make positive changes in their lives and to offer support when needed.

What your week may look like

• Carry out new service user welcomes,
• Ensure that service users have a full understanding of their rights and responsibilities during their stay.
• Support service users to achieve their goals and targets as defined/ agreed within their support plans.
• Manage, update, and communicate service users risk assessments in line with You Matter policy and procedure.
• Ensure service users shape their own support and understand the purpose of the support provided.
• Hold planned key-work sessions each week with service users using a person-centred reflective approach.
• Assess support needs via Person Centred Support Plan, ITEP mapping and plan support around individual need. Regularly review and develop mutually agreed support plans with service users and any other professional involved in the support of those individuals e.g., Social Workers, CPNs, Probation Officers
• Up to date records must be kept on both support planning systems MainStay and DAVE.
• Conduct bi-monthly risk assessments of service users keeping all relevant parties informed of updates/changes.
• Support all service users with future tenancy planning ensuing that everyone has the skills to live independently away from a supported housing pathway.
• Motivate and challenge service users so they realise their goals.
• Liaise between service users and external agencies regarding the provision of specialised support when appropriate.
• Encourage involvement and participation among service users in the running and shaping of the service provided by planning and attending regular service user meetings and forums.
• Deal promptly with all complaints and protection from abuse disclosures.
• Demonstrate creativity in overcoming obstacles and a commitment to working with and providing support to vulnerable people.
