Home Group Limited in Hampstead is seeking a part-time Support Worker to lead planning and coordinate a team to deliver person-centred support. You will balance safeguarding, admin and meetings while guiding colleagues and managing caseloads.

You will help customers with daily tasks, support complex needs, and collaborate with multi-agency teams to safeguard and promote independence, aiming for move-on into the community.

Qualifikationen

• Passion and experience in supporting others ideally in a supported housing setting.
• Experience assessing referrals and coordinating support.
• Ability to work under your own initiative, remain calm under pressure and have a resilient approach.
• Experience supporting customers with complex needs in homelessness and related services.

Aufgaben

• Lead support planning and coordinate our Support Workers to deliver person-centred support.
• Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support Customers with daily tasks like budgeting, cooking and attending appointments.
• Work with multi-agency teams to safeguard and empower our customers.
• Help customers build independence and prepare for move-on into the community.
