Job summary

Creative Support's Doncaster-based Supported Living Service is seeking a caring, dynamic Support Coordinator to coordinate support for 28-30 individuals across 3 locations. You will work with families, external professionals, and staff to ensure high-quality, person-centred care that promotes independence and positive outcomes.

We require a minimum of 3 years' experience with learning disabilities, experience in needs assessments, support planning and risk assessments, and a clean UK driving

Qualifications

• Excellent communication and relationship-building skills.
• Ability to work with families, professionals and staff to coordinate support.
• Organised, resilient and able to manage multiple priorities across services.

Responsibilities

• Coordinate services for individuals across multiple sites.
• Develop, review and monitor person-centred support plans and risk assessments.
• Build positive relationships with the people we support, their families, advocates and professionals.
• Ensure that support plans promote independence and positive outcomes.
• Monitor service quality to maintain high standards.
• Support staff to deliver compassionate, outcome-focused support.
• Identify opportunities for community participation, education, employment and social inclusion.
• Respond to changing needs while promoting positive risk-taking and risk management.
