Job summary

Yorkshire Community Care is seeking a reliable Support Worker (Driver) to deliver person-centred support at clients' homes and in the community across Leeds. You will be responsible for safe transport to appointments and activities, while promoting dignity and independence for service users.

The role requires a valid UK driving licence, strong communication skills, and a compassionate, flexible approach. DBS checks and references are part of pre-employment screening.

Job Overview

We are seeking a reliable and compassionate Support Worker (Driver) to join Yorkshire Community Care’s home support team. The role involves providing person-centred support to adults with additional needs in their homes and community, with responsibility for transporting service users safely to appointments and activities. This is a rewarding opportunity for someone who values dignity, independence and positive outcomes for the people we support.

Key Responsibilities
  • Provide safe, respectful and person-centred support in service users’ homes and community settings in line with individual care plans.
  • Transport service users safely using a company or privately supplied vehicle, ensuring compliance with road safety, vehicle checks and company driving policies.
  • Plan and accompany service users to appointments, social activities and community events, promoting independence and social inclusion.
  • Support with personal care, medication prompts/administration as delegated, meal preparation, household tasks and other daily living activities as required.
  • Maintain accurate and timely records of support provided, including travel logs, incident reports and updates to care plans using organisational systems.
  • Build positive relationships with service users, families and carers, respecting confidentiality, dignity and individual preferences.
  • Identify and report concerns about health, wellbeing or safeguarding promptly to the appropriate manager or external professionals.
  • Participate in team meetings, training, shift handovers and supervision to ensure high standards of care and continuous improvement.
  • Adhere to infection prevention and control procedures, health and safety requirements and all organisational policies.
Person Specification
  • Previous experience in a care, support or community transport role is desirable but not essential; full training will be provided.
  • Valid UK driving licence with no more than the company’s permitted endorsement points; willingness to undertake vehicle checks and comply with driving policies.
  • Have own care and insurance for the work commuting pruposes.
  • Comfortable driving a range of vehicles and able to assist with wheelchair transfers or use of mobility aids where required (training provided).
  • Excellent communication and interpersonal skills with the ability to build positive relationships with people who use services, families and colleagues.
  • Compassionate, patient and flexible approach with good observational skills and the ability to respond calmly in unexpected situations.
  • Ability to follow written care plans, complete records accurately and use basic IT systems for reporting and rostering.
  • Committed to promoting independence, dignity and inclusion for all service users.
  • Willingness to undertake an enhanced DBS check and provide satisfactory references as part of pre-employment checks.
  • Flexible approach to working hours to meet service needs, which may include early starts, evenings or occasional weekends.
