Job summary

Yorkshire Community Care is seeking a compassionate Complex Care Support Worker (Driver) to provide person-centred support to adults with complex needs, including safe transport to appointments and community activities. You will work within a multidisciplinary team to promote independence, dignity and positive outcomes for the people we support.

Qualifications

• Experience supporting adults with complex needs is desirable.
• Valid UK driving licence with clean record.
• Knowledge of safe manual handling and use of equipment.
• Experience managing tracheostomy and ventilation (CPAP).
• Strong communication and organisational skills.
• Willingness to undertake enhanced DBS check and pre-employment checks.
• Flexible with travel between locations and occasional shifts.

Responsibilities

• Provide safe, person-centred support, promoting independence and dignity.
• Safely drive service vehicles to transport service users to appointments and activities.
• Assist with mobilisation, manual handling and safe transfers.
• Support personal care tasks while maintaining privacy and dignity.
• Administer and support medication per policies and care plans.
• Contribute to assessment, care planning and multidisciplinary meetings.
• Maintain accurate records, logs, handovers and incident reports.
• Keep vehicles clean, safe and report maintenance issues.
• Attend mandatory training, supervision and team meetings.
• Support equality, diversity and inclusion in care.
