Job summary

South Gloucestershire and Stroud College (SGS College) at Filton Campus is seeking a Transitions Coordinator to support young learners, including those with SEND, as they move into college life. The role emphasises developing robust transition processes and close collaboration with college teams and external partners to ensure a smooth learner journey.

Qualifications

• Experience working with young people with SEND.
• Proficiency in MS Office suite.
• Ability to negotiate progression opportunities for students.
• Strong organisational, time management and presentation skills.

Responsibilities

• Manage the transition into college for a cohort of complex learners with and without EHCPs.
• Attend EHCP annual reviews and liaise with external stakeholders.
• Collaborate with internal college teams to ensure timely and accurate information transfer.
• Develop SGS's transition process to ensure a robust start to the learner journey.
• Actively seek, record and act on feedback from learners, parents/carers and other stakeholders.
