Job summary

DN Colleges Group is recruiting an Inclusive Transition Coordinator to join our Student Services/SEND team in Scunthorpe. You will support learners with SEND as they transition into college, coordinating personalised plans and liaising with schools, families, curriculum teams and external agencies.

This rewarding role focuses on inclusion, independence and ensuring every learner can progress. You will manage a caseload, track progress and contribute to EHCPs and transition planning.

Qualifications

• Level 2 or Level 3 qualification in learning support, SEND, education, youth work or equivalent experience.
• Level 2 English and Maths qualifications (or equivalent).
• Experience of supporting young people with SEND in an education, training or related environment.
• Knowledge of SEND and EHCPs, with ability to develop person-centred support plans.
• Excellent communication, organisational and relationship-building skills, with the ability to manage a varied caseload.

Responsibilities

• Work with schools to support prospective learners with SEND as they transition into college.
• Develop personalised, person-centred transition plans.
• Manage a caseload of learners and monitor progress against actions.
• Contribute to EHCP consultations, reviews and transition planning.
• Liaise with learners, parents/carers, curriculum teams, support services and external agencies.
• Coordinate transition activities, including visits and information-sharing meetings.
• Ensure learner support info is communicated accurately to college teams.
• Maintain confidential records in line with data protection requirements.
• Promote inclusion, independence and positive progression outcomes for learners with SEND.
• Support preparation for adulthood outcomes including employment and independent living.
