Job summary

Barts Health NHS Foundation Trust is seeking a Band 4 Locum General Rehabilitation Support Worker for the London area. You will join the Community Stroke Team and support patients recovering from strokes in their home environments, contributing to rehabilitation and overall well-being.

Responsibilities include delivering rehabilitation programmes, assisting with daily activities, manual handling, and monitoring patient progress.

Qualifications

• Experience as a rehabilitation support worker is essential.
• Comfortable with lone working in the community and able to travel around Tower Hamlets.
• Physically capable of manual handling at patients' homes.
• Strong communication and interpersonal skills for patients and families.

Responsibilities

• Assist in delivering rehabilitation programmes as directed by the physiotherapist or occupational therapist.
• Support patients in daily activities to regain independence and improve quality of life.
• Perform manual handling and mobility assistance to ensure safety and comfort.
• Monitor patient progress and report changes to the clinical team; document interactions and progress.
• Work effectively in the community across Tower Hamlets.
