Job summary

Derbyshire Community Health Services NHSFT is seeking a Speech and Language Therapist to join the ESSD and Adult Community SLT teams across South Derbyshire, Amber Valley and Erewash. You will support early hospital discharge and provide home-based stroke rehabilitation within a strong, multidisciplinary service.

Qualifications

• Recognised Speech and Language Therapy degree qualification.
• Post-registration experience including dysphagia, stroke and long-term conditions.
• Registered HCPC member.
• Ability to move between community locations during the day.

Responsibilities

• Lead and influence service provision and clinical pathways for ESSD and SLT.
• Deliver high-quality, evidence-based stroke rehabilitation at patients' homes.
• Work across South Derbyshire, Amber Valley and Erewash in a community setting.
