Job summary

The Stroke Association is seeking a Support Coordinator to assist stroke survivors and families in the Cardiff and Vale area, providing home-based community visits and group activity support.

The role involves frequent travel across the locality to visit people at home, attending team and other work-related meetings, and reporting to the Service Delivery Coach in Wales. This permanent position is 21 hours per week, with funding in place until October 2027.

Responsibilities

• Supporting stroke survivors and families in the community and in group settings with their stroke recovery, rebuilding confidence.
• Reporting to the Service Delivery Coach in Wales, you will have the ability to support Stroke Survivors to identify their goals and maximise their recovery through a person-centred approach.
