Job summary

Stroke Association is recruiting a Support Coordinator to work from home in the Cardiff and Vale area. The role involves frequent travel to visit clients at home and in community settings, plus delivering and coordinating a weekly communication and peer support group.

You will work with NHS colleagues to support recovery and maintain CRM records. The position is permanent, 21 hours per week, with salary around £17,000 per annum.

Qualifications

• Experience in health or social care preferred.
• Ability to visit homes and travel across Cardiff and the Vale.
• Excellent IT and CRM record-keeping skills.

Responsibilities

• Plan and facilitate a large weekly Communication and Peer Support Group.
• Visit stroke survivors at home and in community settings.
• Collaborate with NHS colleagues and community services.
• Maintain accurate records on a CRM database.
• Support stroke survivors to identify goals and maximise recovery.
