Dorset HealthCare University NHS Foundation Trust is seeking a Stroke and Neuro Navigator to support people in the community across the West and North of Dorset. You will provide advice, signposting and person-centred interventions, and lead or support groups as needed.

You will join a multidisciplinary team and may work across home visits, community settings and a base location, with hybrid working and a flexible caseload designed to promote independence and better outcomes for people affected

Qualifications

• Be responsible for a specific caseload in Pathway 1 (advice, support and signposting)
• To work effectively as a member of the Stroke and Neuro Service
• Provide a flexible, client centred intervention providing personalised information and support to patients and carers/family on the DSNS caseload
• To work with people on pathway 1 to identify goals and contribute to the care planning process
• To link patients /carers with various voluntary sector organisations including Stroke Association and use of digital support platforms
• Navigating and assisting referrals to health and well-being resources/services - Live Well Dorset, Social prescribers, leisure sector and support groups
