Job summary

Herefordshire and Worcestershire Health and Care NHS Trust is seeking an enthusiastic stroke specialist nurse to lead the stroke nursing team. The post involves specialist assessments, needs-based treatment, and flexible working patterns.

You will predominantly manage a caseload with MDT support, promote secondary prevention, and support stroke survivors toward independent, meaningful lives. The role offers development opportunities and a robust induction programme.

Qualifications

• Two years' nursing experience
• Experience of working with stroke patients and rehabilitation nursing
• Staff supervision
• Resource management

Responsibilities

• Provide holistic assessment and evidence-based nursing care to stroke patients to enable them to achieve their best potential.
• Demonstrate stroke knowledge to educate patients regarding healthier lifestyles, blood pressure management and psychological support.
• Supervising/supporting the Band 3 Rehabilitation Assistants and working within an interdisciplinary team.
• Contribute to the development and evaluation of the nursing service.
• To act as a source of expertise on stroke patients management.
• Liaising with GPs, Stroke Consultants and Third Parties.
• Maintain accurate documentation in line with guidelines and trust requirements.
