Job summary

Dorset HealthCare University NHS Foundation Trust is seeking a Stroke and Neuro Navigator to support community and home-based patients in the West and North of Dorset. The role focuses on advice, signposting, and personalised information within a multidisciplinary Stroke and Neuro Service.

The post offers hybrid working with a mix of community, base and home-based caseloads, and requires driving and right to work in the UK. The closing date is 19 November 2026; sponsorship is not available.

Qualifications

• Good communication skills (written and verbal).
• Ability to work unsupervised within a supportive environment.
• Ability to work as part of a team.
• Ability to manage time/caseload pressures in a rapidly changing environment.

Responsibilities

• Be responsible for a specific caseload in Pathway 1 (advice, support and signposting).
• Provide a flexible, client-centred intervention providing personalised information and support to patients and carers/family on the DSNS caseload.
• Link patients/carers with voluntary sector organisations including Stroke Association and use of digital support platforms.
