Herefordshire and Worcestershire Health and Care NHS Trust is seeking a Junior Sister/Charge Nurse to lead the stroke nursing team. The role focuses on specialist assessment, needs-based treatment for stroke patients, and secondary prevention education for survivors and families.

You'll work in an interdisciplinary rehabilitation setting, supervise Band 3 Rehabilitation Assistants, attend weekly MDT meetings, and support patients with morning routines, medicines and meals to promote

Qualifications

• Current NMC registration required.
• RN / RGN plus nursing degree or diploma.
• Evidence of ongoing continuing professional development.
• Two years' nursing experience, including stroke care.

Responsabilités

• Carry out comprehensive assessments of stroke patients and manage a caseload with specialist treatment.
• Provide holistic, evidence-based nursing care to help patients reach their best potential.
• Assess risk factors and deliver secondary prevention education on lifestyle changes and BP management.
• Supervise Band 3 Rehabilitation Assistants within an interdisciplinary team.
• Attend weekly MDT meetings and liaise with GPs, stroke consultants and third-party services.
• Support morning routines, medicines and meal preparation to promote independence.
• Contribute to development and evaluation of the nursing service and maintain records.
