Herefordshire and Worcestershire Health and Care NHS Trust is seeking an enthusiastic stroke specialist nurse to lead the stroke nursing team. The post involves specialist assessments, needs-based treatment, and flexible working patterns.
You will predominantly manage a caseload with MDT support, promote secondary prevention, and support stroke survivors toward independent, meaningful lives. The role offers development opportunities and a robust induction programme.
An excellent opportunity has arisen for an enthusiastic, stroke specialist nurse to lead our stroke nursing team. The ability to use your initiative, work proactively, demonstrate excellent communication and influencing skills, manage and prioritise your own workload and respond flexibly to changing demands are essential requirements of the post.
You will predominantly manage a case load performing specialist assessment and needs-based treatment. The role will provide you with variety and no two days are the same. As a trust we offer a robust induction period and continue to promote opportunities for development through 1:1 supervision session, peer working and in-service training. The trust offers regular training opportunities via Learning and Development to promote your clinical and non-clinical skills. This is a full time role but part time would be considered. Applicants must read the attached guidance notes before submitting your application for full understanding of Trust requirements.
Day to day, you will be undertaking comprehensive assessments of mild, moderate and severe stroke patients including those with multiple pathologies/co-morbidities. You will be supported clinically by the Clinical Stroke Nurse Lead and established senior Stroke Nurses to be able to assess risk factors for stroke, work with stroke survivors and their families in giving secondary prevention education whilst promoting patient advocacy and empowerment. You will also be actively supporting stroke survivors with any altered mood issues to enable stroke survivors to lead purposeful and fulfilling lives. You will attend the weekly MDT in which the team will discuss the progress of patients and identify goals that are achieved and ongoing. You will also need to liaise with other health care professionals within the Trust/Acute Trust and third party agencies such as The Stroke Association. We pride ourselves on being truly interdisciplinary, so you will also be involved with supporting patients to undertake their morning routine, support with medications and/or facilitate their meal preparation to promote independence. These sessions are part of a rehabilitation focused approach of which we can evidence great results in patients regaining independence. You will be given the appropriate training and support to do this.
