Job summary

Locala Health & Wellbeing is recruiting a Staff Nurse (Band 5) for 37.5 hours per week on a permanent basis. The post supports the Intermediate Care Team, focusing on complex wound care, monitoring deteriorating patients and facilitating safe discharge from hospital to home.

The Nursing Team collaborates with GPs, Physician Associates, OT, PT, Pharmacists and our Kirklees Care Team to deliver high-quality, person-centred care in a supportive environment. DBS checks may be required.

Qualifications

• Qualified nurse with registration in the UK
• Experience in intermediate care or related clinical setting preferred
• Must be able to demonstrate right to work in the UK and DBS clearance

Responsibilities

• Provide high-quality nursing care to service users in the Intermediate Care Team.
• Monitor patient condition and respond to deterioration as part of a multidisciplinary team.
• Participate in discharge planning and support patients returning home.
• Collaborate with GPs, Physician Associates, OT, PT, Pharmacists and support workers to optimise care.
