Job summary

Locala is recruiting a Staff Nurse to join our Intermediate Care Team in the Kirklees area. The role is Band 5 with a salary of £32,073-£39,043 and 37.5 hours per week on a permanent basis, delivering high-quality nursing care during patient discharge and home recovery.

You will work collaboratively with GPs, Physician Associates, and other professionals to support service users on their journey home, monitor deteriorations, manage complex wound care, and uphold the trust's standards.

Responsibilities

We are looking for a highly motivated and enthusiastic Staff Nurse to join our Intermediate Care Team.

The Intermediate Care Service supports medical and surgical service users from discharge from hospital, to their journey home. Every day there is something different from complex wound care, to monitoring deteriorating patients and so much more in between.

The Nursing Team boasts a variety of skills and experiences to enhance your knowledge and professional development within a supportive environment.We pride ourselves onexcellentstandards of Nursing Care and service user satisfaction.

The Nursing Team works collaboratively with the multi-disciplinary team, compromising of GPs, Physician Associates, Occupation Therapists, Physiotherapists, Pharmacists, Healthcare Support Workers and our Kirklees Care Team. Together we rehabilitate service users to succeed in their home environment, preventing hospital admissions and preserving safety.
