Job summary

You will liaise with GPs, Local Authority partners and employers, while handling data securely under UK GDPR and contributing to monthly KPI reporting and service improvement initiatives.

Responsibilities

• Receive, acknowledge and log referrals to the WorkWell SPOA from all sources in accepted formats.
• Conduct initial non-clinical screening of referrals using the triage protocol and escalate any risk signals.
• Allocate accepted referrals to local Work and Health Coaches and track contact and first assessment timelines.
• Arrange warm handovers and coordinate cross-border referrals with Suffolk WorkWell SPOA.
• Maintain accurate electronic participant records and apply SNOMED codes.
• Prepare and submit management information, including KPI reports and activity logs.
• Act as first point of contact for enquiries and maintain the directory of onward referral partners.
