Job summary

Norfolk Primary Care CIC is seeking an experienced Advanced Nurse Practitioner to chair the WorkWell MDT and lead clinical triage within the SPOA. You will oversee complex triage decisions, ensure timely safety assessments and coordinate handovers with GPs and health partners.

Qualifications

• Registered Nurse with current NMC registration.
• Master's degree (Level 7) in Advanced Clinical Practice or equivalent.
• Non-medical independent prescriber (V300).
• Post-registration qualification in clinical assessment, history taking and decision making.
• Safeguarding adults and children training Level 3.
• Fit Note and Work and Health e-learning (e-LfH) or willingness to complete.
• Post-registration qualification or training in mental health assessment.
• Occupational health nursing or vocational rehabilitation qualification.
• Health coaching or motivational interviewing training.

Responsibilities

• Lead clinical triage for the Norfolk WorkWell SPOA and triage referrals within one working day.
• Provide clinical oversight of triage decisions and review complex cases.
• Assess safety risks (including suicide risk) and determine WorkWell suitability.
• Coordinate warm handovers when WorkWell is not suitable.
• Chair the WorkWell MDT and guide case discussions and decisions.
• Lead MDT decisions on input, referrals, escalation and discharge.
• Certify Med3 fit notes and supervise staff in the MDT.
• Develop and review triage protocols and escalation pathways.
• Ensure governance, safety and quality through audits and reporting.
• Foster collaboration with GP practices, PCNs and partner organisations.
