Job summary

Milton Keynes University Hospital NHS Foundation Trust is seeking a Specialist Pharmacist in Frailty and Care of the Elderly to provide expert clinical pharmacy services for older adults. You will conduct comprehensive medication reviews, optimise therapy, and support safe discharges while working with a multidisciplinary team in a modern, expanding hospital.

The role offers leadership opportunities, supervision of junior staff, and involvement in service development within Frailty and Elderly

Qualifications

B.Sc./B.Pharm./M.Pharm and GPhC registration.
Postgraduate Certificate in Clinical Pharmacy (or equivalent).
Evidence of on-going CPD (in line with GPhC requirements).
Postgraduate Diploma in Clinical Pharmacy or GLF evidence.
Member of RPS.
Knowledge of medicines management at interfaces between primary, secondary and tertiary care.

Responsibilities

Provide expert clinical pharmacy services to older adult inpatients.
Undertake comprehensive medication reviews and optimise medicines.
Reduce inappropriate polypharmacy and improve adherence.
Support safe discharges and transfer of care for the elderly.
Lead and supervise junior pharmacists and technicians; champion medicines optimisation.
Contribute to quality improvement and audits; participate in pharmacy out-of-hours service.
