Job summary

Milton Keynes University Hospital NHS Foundation Trust is seeking a Specialist Pharmacist for Frailty and Care of the Elderly to provide ward-based clinical pharmacy services within the elderly care pathway and support medicines optimisation and patient safety initiatives.

You will develop leadership and supervisory skills, contribute to quality improvement and audits, and support the pharmacy service including out-of-hours activities to deliver outstanding patient care.

Qualifications

• Ward-based clinical pharmacy services within Frailty and Care of the Elderly.
• Leadership and supervision of junior pharmacists and pharmacy technicians.
• Quality improvement and medicines safety initiatives.

Responsibilities

• Deliver ward-based clinical pharmacy services to Frailty and Care of the Elderly patients.
• Provide clinical support to the dispensary and engage in service development.
• Lead or participate in audits, medicines optimisation projects and safety initiatives.
