Qualifikationen

• Deliver structured medication reviews for frailty patients, focusing on complex polypharmacy.
• Identify opportunities for deprescribing considering treatment burden and patient goals.
• Work with the Ageing Well MDT to support personalised care planning.

Aufgaben

• Manage medicines management plan with the Lead Clinical Pharmacist and with PCN policies.
• Collaborate with PCN pharmacists, clinicians, and ICB to optimise prescribing and safety.
• Provide medication review services in clinics, domiciliary visits, and residential/nursing homes, including remote options.
• Produce quarterly practice performance reports and maintain accurate patient records.
• Support development of new workflows to improve prescribing quality.
