Job summary

Havant & Waterlooville Primary Care Network is seeking an Ageing Well Clinical Pharmacist to join the multi-disciplinary Ageing Well Team based at Bosmere Medical Centre, Havant. The post focuses on medicines optimisation for frailty, specialist reviews, deprescribing, and reducing medicines-related harm.

You will work with GPs, Practice Managers and the wider health and social care teams to deliver person-centred care.

Qualifications

• Evidence of CPPE GP, MOCH or PCN pathway engagement.
• Qualified Prescriber status and current GPhC registration.
• Full UK Driving Licence with access to transport.
• Experience in medicines optimisation for frailty and polypharmacy.

Responsibilities

• Provide expertise in clinical medicines management.
• Carry out person-centred structured medication reviews for frailty patients.
• Collaborate with Ageing Well team to support personalised care planning.
• Support management of long-term conditions and safe prescribing.
• Oversee medicines on transfer of care and implement safety systems.
• Manage repeat prescriptions and respond to acute requests.
