Job summary

Slough Borough Council is hiring 2 experienced Occupational Therapists to join the Long Term Occupational Therapy Team in Slough. You will deliver person-centred interventions to adults with complex and long-term needs, promoting independence and reducing reliance on long-term care.

You will work with multidisciplinary teams on DFG referrals, Assistive Technology and Discharge to Assess pathways, contributing to service improvements and professional development within a collaborative, supported

Responsibilities

• Lead on Disabled Facilities Grant referrals and environmental adaptations.
• Promote Assistive Technology and digital tools like AskSARA to enhance independence.
• Support Discharge to Assess pathways, Proportionate Care and In-Reach projects with residential settings and domiciliary care providers.
• Collaborate with Integrated Care and Decision Making (ICDM) clusters, GPs and community health professionals.
