Job summary

Slough Borough Council is hiring two experienced Occupational Therapists to join the Long Term Occupational Therapy Team. You will deliver high-quality, person-centred interventions to adults with complex needs in the community, promoting independence and reducing long-term care reliance.

As a Specialist Occupational Therapist, you’ll work with multidisciplinary teams, lead on environmental adaptations, and support discharge pathways.

Qualifications

• Specialist Occupational Therapist with experience in long-term adult care.
• Ability to work within multidisciplinary teams to deliver patient-centred interventions.

Responsibilities

• Lead on Disabled Facilities Grant referrals and environmental adaptations.
• Promote use of Assistive Technology to enhance independence.
• Support discharge to assess pathways and collaborative care projects.
• Collaborate with GPs, ICDM clusters, and community health professionals.
