Job summary

Central London Community Healthcare NHS Trust is seeking a Specialist Occupational Therapist (Band 6) for the MeHBRT in Merton. You will work in a multidisciplinary home-based rehab team, providing timely, person-centred OT to patients transitioning from hospital with ongoing needs.

You will assess, plan and implement rehab interventions, manage a caseload, and contribute to service development, quality and training of junior staff.

Qualifications

• Degree or Diploma in Occupational Therapy.
• Current AHP with UK HCPC registration.
• Post-qualifying experience in elderly, neurology and community rehab.
• Experience in line managing/supervision of staff.

Responsibilities

• Provide specialist assessment, rehabilitation and clinical leadership within MEHBRT.
• Assess referrals for discharge with ongoing health or social care needs.
• Manage a caseload including complex cases with senior support as needed.
• Develop and evaluate OT interventions to promote independence and safe transitions home.
• Provide clinical leadership and supervision to the MDT and students.
• Contribute to service quality, performance and continuous improvement.

Job overview

Join our Merton Enhanced Home-Based Rehabilitation Team (MEHBRT) as a Specialist Occupational Therapist and help people make meaningful progress towards recovery and independence in their own homes.

Working as part of a multidisciplinary team, you will provide timely , person-centred occupational therapy to patients transitioning from hospital who have ongoing health or social care needs.

This is an opportunity to combine specialist clinical practice with leadership and service development. You will:

Assess patients and develop rehabilitation plans that reflect their individual needs and goals.

Support the development, delivery and evaluation of the MEHBRT service.

Manage your own caseload, including complex cases, with access to senior support when needed.

Provide clinical leadership, advice, education and supervision to colleagues and students.

Contribute to service quality, performance and continuous improvement.

You will work collaboratively with patients, families, carers and colleagues across the multidisciplinary team to support safe, effective rehabilitation and positive outcomes.

Main duties of the job

You will provide specialist assessment, rehabilitation and clinical leadership within the MEHBRT.

Assessing referrals from hospitals for patients who are ready for discharge but require ongoing health or social care support.

Managing an individual caseload, including patients with complex needs, seeking senior support where appropriate.

Developing, implementing and evaluating occupational therapy interventions that promote independence and support safe transitions home.

Contributing to the development and ongoing evaluation of the MEHBRT service.

Providing clinical leadership and advice to Physiotherapists, Rehabilitation Support Workers and the wider multidisciplinary team.

Providing education and supervision to Rehabilitation Support Workers and multidisciplinary students.

Line managing and supervising junior staff within the service.

Supporting the team to achieve agreed performance indicators and maintain high standards of quality and patient care.

Working collaboratively across professional teams to ensure a coordinated and seamless service for patients.

You will use your clinical expertise , professional judgement and communication skills to support positive outcomes for patients and contribute to the continued development of the service.

Qualifications

• Degree or diploma in Occupational Therapy
• Current AHP with UK HCPC registration
• Up to date evidence & relevant knowledge through CPD as evidenced by portfolio
• Membership of Royal College of Occupational Therapy and / or relevant special interest groups
• Attendance at Clinical Educator Course

Experience

• Appropriate post-qualifying experience in a range of areas acute care of the elderly, neurology and community
• Appreciation of constraints to working in the community
• A clear understanding of current NHS issues, professional standards, CPD, clinical governance and experience of audit
• Ability to assess patient conditions, establish the patients problem, formulate and implement a treatment plan
• Has sound knowledge of the Mental Health Act and other legislation
• Experience of MDT working and understand their roles
• Some experience of supervising juniors/assistant/students
• Some experience in research / literature reviews
• Clinical Teaching
• Experience in community work within rehab
• Post registration experience at band 5 level in a wide range of clinical areas including elderly rehabilitation, community rehabilitation, falls management, dementia care, respiratory care and orthopaedics

• Experience of line managing / Supervision of staff

Skills

• Knowledge and understanding of National and local frameworks and guidelines for older people.

• Highly developed specialist knowledge and application of assessments and interventions relevant to client group
