Job summary

Central London Community Healthcare NHS Trust is seeking a Specialist Occupational Therapist (Band 6) for a fixed-term maternity cover at Edgware Community Hospital. You will assess, plan and deliver specialist OT care to older people living with frailty and long-term conditions within a supportive MDT.

You will lead on personalised interventions, collaborate with GPs, social care and rehabilitation services, and supervise junior staff while contributing to service development and governance.

Qualifications

• Diploma/degree in Occupational Therapy.
• Current HCPC registration.
• Attendance at specialist CPD activities in Adult Rehabilitation, Falls, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation or Palliation.

Responsibilities

• Conduct comprehensive OT assessments for patients with complex needs.
• Develop and deliver personalised OT treatment and rehabilitation plans.
• Assess and recommend equipment, adaptations and moving & handling solutions.
• Support independence, function and quality of life.
• Work within a multidisciplinary team to coordinate care plans.
• Provide OT guidance to patients, carers and health/social care professionals.

Join our innovative Community Ageing Well Service (CAWS) and make a meaningful difference to older people living with frailty, complex health needs and long-term conditions.

As a Specialist Occupational Therapist, you will work within a supportive multidisciplinary team to help people remain independent, safe and confident in their own homes. You will use your specialist skills to assess needs, provide person-centred occupational therapy interventions and help people achieve what matters most to them.

CAWS provides proactive, preventative and holistic care, working closely with GPs, community rehabilitation, social care, care homes, voluntary sector organisations and Integrated Neighbourhood Teams.

Main duties of the job

As a Specialist Occupational Therapist, you will:

• Complete comprehensive occupational therapy assessments for people with complex physical, cognitive and functional needs.
• Develop and deliver personalised treatment and rehabilitation plans based on individual goals and strengths.
• Assess and recommend appropriate equipment, adaptations and moving and handling solutions.
• Support people to maximise their independence, function, confidence and quality of life.
• Work collaboratively within a multidisciplinary team to develop coordinated, person-centred care plans.
• Provide specialist occupational therapy advice to patients, carers and other health and social care professionals.
• Contribute to falls prevention, frailty management and admission avoidance initiatives.
• Provide supervision, guidance and support to therapy assistants and junior colleagues, where appropriate.
• Contribute to service development, quality improvement and clinical governance.
• Build effective working relationships across health, social care and voluntary sector services to support joined-up care.

Education/Qualification

• Diploma/degree in Occupational Therapy
• Current HCPC registration
• Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Falls, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation, Palliation

Experience

• Substantial post registration experience as a Band 5 Occupational Therapist
• Experience working at a Junior level in a wide range of specialised clinical areas, providing care to frail and elderly patients with long-term conditions and/or complex health needs including outpatients, neurology, rehabilitation, orthopedics and respiratory
• Experience of negotiating and liaising within a multi-disciplinary team in NHS and social services

Skills & Knowledge

• Skills in the assessment and treatment d, identifying any risk or potential problem and act appropriately effective record keeping, with adults with complex conditions and multi-pathologies
• Knowledge and understanding of complex medical conditions including multipathologies and falls.
• Ability to apply core Occupational Therapy skills across a range of clinical conditions within a client centred, goal setting.
• Knowledge of outcome measures and evidence based practice associated with Occupational Therapy
• Good communication and report writing skills, in both verbal and written format to accurately report and feedback information to colleagues and others
• Knowledge of the role of Social Services and voluntary agencies
• Knowledge of research and audit processes
• Participation in research projects
