The post is based in Central Halifax with expectation of covering across Calderdale as required.

• Lead a district nursing team within a geographical area; 24-hour service provision.
• Foster a professional nursing culture open to change and innovation.
• Keep up to date with nursing developments and attend essential training.
• Ensure nursing practice is based on current evidence and best practice.
• Develop mechanisms to support staff in expanding their scope of practice.
• Encourage a learning environment with education and training opportunities.
• Support staff through clinical supervision and accountability.

Calderdale and Huddersfield NHS Foundation Trust are inviting applications for a highly motivated Band 7 Specialist Community Practitioner to provide caseload management and hold responsibility for the care of a defined client group over a 24 hour period.

The post is full time and whilst situated in Central Halifax, there is an expectation that cover would be provided across the Calderdale geographic area where required.

We welcome applications from highly motivated dynamic individuals who currently hold a District Nursing qualification or are currently due to qualify this year.

The post

• To provide professional leadership and carry the continuing responsibility for the assessment of care needs.

• To manage a district nursing team within a geographical area. Calderdale and Huddersfield Foundation Trust provide a 24 hour district nursing service
