The post is based in Central Halifax with expectation of covering across Calderdale as required.

Responsibilities

• Provide professional leadership and carry continuing responsibility for care assessment.
• Manage care for a defined client group (caseload) over a 24-hour period.
• Lead a district nursing team within a geographical area; 24-hour service provision.
• Foster a professional nursing culture open to change and innovation.
• Keep up to date with nursing developments and attend essential training.
• Ensure nursing practice is based on current evidence and best practice.
• Develop mechanisms to support staff in expanding their scope of practice.
• Encourage a learning environment with education and training opportunities.
• Support staff through clinical supervision and accountability.
