Job summary

East Sussex Healthcare NHS Trust is seeking an experienced District Nurse with SPQ to become a Clinical Lead and drive development of a 7-day, high-quality community nursing service. You will provide clinical leadership, mentor staff, and work with operational leads to ensure safe and sustainable delivery of care in patients' homes.

Qualifications

  • RGN/RN (Adults) on NMC Register.
  • Community Specialist Practitioner Qualification (District Nurse).
  • Non-Medical Prescriber V300 (or commitment to undertake).
  • Teaching and assessing qualification aligned to NMC requirements.
  • Advanced Physical Assessment Module (or commitment to undertake within 12 months).
  • Leadership/Management Training (or commitment to undertake).
  • IOSH (or commitment to undertake).
  • Extensive SPQ community nursing experience.

Responsibilities

  • Provide clinical leadership and line management support to a Community Nursing team.
  • Assess new and complex patients in their own homes; plan, implement, and evaluate nursing interventions and care plans.
  • Support delivery of the Enhanced Health in Care Homes Framework.
  • Lead quality improvement initiatives and contribute to clinical governance and caseload management.
  • Support patients and carers to achieve maximum independence through evidence-based practice while adhering to policies.

Job description

Are you a District Nurse with the Specialist Practice Qualification (SPQ) looking for the opportunity to develop your career? If you are, an exciting opportunity has arisen for an ambitious and enthusiastic Clinical Lead Nurse to join our District Nursing Service. We are looking for District Nurse who holds a Community Specialist Practitioner Qualification with significant Community Nursing experience to lead a Primary Care Network (PCN) aligned Locality team. The post holder will need to demonstrate a comprehensive specialist knowledge base and promote excellence in nursing practice in a dynamic healthcare environment. They will be responsible for the development and delivery of a 7 day consistently high quality, safe and sustainable, cost efficient service operating 08:00 till 18.00Main duties of the job You will provide clinical leadership and line management support to a Community Nursing team, working closely with operational leads to deliver high-quality patient care. The role includes assessing new and complex patients in their own homes, planning, implementing, and evaluating nursing interventions and care plans. You will support delivery of the Enhanced Health in Care Homes Framework in line with local service objectives. You will lead continuous quality improvement initiatives and contribute to effective clinical governance and caseload management within the team. In addition, you will support patients and carers to achieve maximum independence through evidence-based practice while adhering to national and Trust policies and guidelines.

  • RGN/RN (Adults) on NMC Register
  • Community Specialist Practitioner Qualification (District Nurse)
  • Non-Medical Prescriber V300 (or commitment to undertake)
  • Teaching and assessing qualification aligned to NMC requirements
  • Advanced Physical Assessment Module (or commitment to undertake within 12 months of coming into post)
  • Leadership/Management Training (or commitment to undertake)
  • IOSH (or commitment to undertake), Extensive SPQ community nursing experience
  • Teaching in a clinical setting
  • Experience of multidisciplinary working
  • Evidence of up to date knowledge and skills
  • Significant experience of team leadership and organisational skills at a senior level
  • Experience of caseload management, Experience of managing change and developing teams
  • Long term condition management
  • Motivational Interviewing, Independent prescribing, medication review and administration
  • Skilled in the use of reflective practice and proactive clinical supervision, Demonstrates a sound understanding of community developments and priorities both locally and nationally
  • Skills in staff appraisal
  • Ability to facilitate innovative practice, Ability to travel easily in mixed geographical areas ensuring a timely delivery of patient care
  • Must be able to safely carry essential equipment and provide transport to others involved in community nursing business e.g. students/colleagues
