This clinically focused role includes occasional dispensary cover and supports the out-of-hours medicines advisory service and weekend working (9.30am-1.30pm), maintaining a minimum 1 in 12 rota.

Main duties of the job
  1. Be a valued member of the Medicines Management team and a core member of multidisciplinary teams providing a clinical pharmacy service to in-patient mental health services.
  2. Help deliver and develop ward-based medicines management activities to promote the effective and efficient use of medicines, improving patient care and minimizing waste.
  3. Work as part of the Medicines Management team and be a core member of the inpatient multidisciplinary team providing a clinical pharmacy service to in-patient areas.

Job description

Main responsibilities include:

  1. Implementing the Medicines Management strategy within the Trust.
  2. Deputising for a higher grade pharmacist as required.
  3. Participating in rota at a specialist level, including Medicines Information and providing professional and legal supervision to dispensary services.
  4. Contributing to the Trust-wide Out of Hours Medicines Advice Service and 7-day supply service.
  5. Completing medicines reconciliation following admission or transfer, collating information from patients and liaising with healthcare professionals to ensure safe and effective medication use.
  6. Ensuring medicines have appropriate indications and therapies are suitable for diagnoses.
  7. Verifying that existing therapies are safe and have not contributed to presenting complaints.
  8. Ensuring new treatments are compatible with patients' conditions.
