Oxford Health NHS Foundation Trust is seeking an experienced healthcare professional to cover maternity leave in the Single Point of Access team, central entry point for community physical health services.

The role involves triaging referrals from patients, GPs and other professionals, managing clinical risk, directing to appropriate pathways, and coordinating shifts and staff line management in a demanding 8am–8pm, 365-day service. A full driving licence and vehicle access are required.

Formación

• Registered health professional qualification.
• Degree level education or equivalent.

Responsabilidades

• Triage referrals from patients, GPs and other healthcare professionals using clinical assessment.
• Manage clinical risk and direct patients to the most appropriate service, including Urgent Community Response, district nursing, community therapy, falls, palliative care, community hospitals, crisis and social care.
• Coordinate service delivery and shift coordination.
• Line manage and support staff.
• Contribute to staff development.
• Contribute to continuous service improvement.
• Work collaboratively with clinical and administrative colleagues.
